Registration

Affiliation - Your affiliation paperwork and fees ($25.00 for an exiting affiliate and $30.00 for a new affiliate) are due June 1. This includes a listing of your officers with names, addresses, telephone numbers, and email addresses. We will also need a copy of your constitution, bylaws and operational rules. This form can be found on our web site under “FORMS”
- Affinity database is now open so you can start registering your players. A Submission of Registration form should be used when sending in payment for your players. This form must accompany each payment you send into the office so we can keep our records accurate. This form can be found on our web site under “FORMS”
- Player Release and Disclaimer Form – This form will not be needed this any longer. This same information will be on the back of their player pass.
- Certificate of Insurance Request – As soon as you know which fields you will be playing on please send in your Certificate of Insurance Request. This form can be found on our web site under “FORMS”.
- Calendar - If your league has a calendar indicating registration dates, team formation dates and play dates please send it in to ASA so we can be sure all the paper work is ready for you to start play.
